What Is the Rain Shift?
The Rain Shift is a special weather shift introduced in the Aug 8, 2026 update. It shares the name with the July Hazmat Team slime event, but is a separate mechanic: when it starts raining, a new type of patient begins coming to the hospital.
The rain is not cosmetic. Flood water and slippery patches appear inside the facility, new fish-shaped, aquatic-styled patients check in at reception, and some of those aquatic patients turn out to be underwater anomalies.
How to Trigger the Rain Shift
- Reach Shift 3 or later: The Rain Shift is a Shift 3+ special shift that can happen any round after that point.
- Watch the weather: When it begins to rain outside, the special rain shift starts. Rain is clearly visible through windows and at reception.
- Expect aquatic patients: Once the rain starts, new fish and aquatic patients begin arriving at check-in for the rest of the shift.
Fish & Aquatic Patients
Fish-Shaped Patients
Fish with underwater designs, including fish with tails, spiky fins, and aquatic styles unlike any normal patient. They check in like normal animals but look distinctly different.
Shark-Like Patients
Shark-inspired visitors with tails and paddled shapes — some with horns on their heads. They talk, so expect audio clues like any other patient.
Angler-Fish Patients
Angler-fish styled patients with glowing lure features and tails. These are the fish most clearly tied to real marine animals.
Aquatic Swims with Other Fish
Multiple aquatic variants can spawn in the same rain shift — every fish is unique, and even the water itself can highlight anomalies behind the patients.
Underwater Anomalies
The water, aquatic patients are not always what they seem. "They can also be anomalies," — the underwater patients are candidates for dark variants. An anomaly that looks like a fish may walk, and snarling turn on you in the treatment room.
Use the same detection loop as normal: check visual signs (looking for missing faces, fake fins, and odd colors) at check-in, photograph the patient with the camera, review it, and compare on CCTV before admitting anyone.
Rain Floor Hazards

- Scan fish patients at reception: Photo them before admitting so anomalies can't sneak inside to be treated.
- Keep your sanity high: the Rain Shift adds movement slowdown and new threats — don't risk dipping below 50% sanity.
- Route around puddles: if flung patches slow you down, use shift changes to reposition rather than stepping through the water hazard.
