Always check item boxes carefully before looting. If the box shakes or makes noise, use Decontamination Spray on it before opening.

Tiny Anomaly (Hazmat Update)

A miniature patient (commonly a pink rabbit with three eyes) that checks in at reception. Currently all tiny visitors are anomalies — none are legitimate patients.

Counter

Close the shutters to refuse service. If it gets inside, it chases and bites the nearest player, draining sanity. Use a taser or gun to defeat it quickly.

Rule of thumb: if a patient is shockingly small, it is the baby anomaly. Snap a photo and confirm the three eyes before the shutters go up — a false alarm is always cheaper than a Tiny Anomaly loose in the ward.

Special Monsters

Head Banger

A purple bear-like patient that smashes its head against the reception window, blocking the shutters from closing.

Counter

Brew a coffee, take two sips, and bring it to the window. Use the E prompt to offer the coffee. The Head Banger drinks it and leaves peacefully.

Bed Monster

Long black arms reach out from underneath hospital beds, threatening to drag patients or players under.

Counter

Watch for black arms near bed areas. Stay out of the red danger zone. Feed it Maple Syrup to make it disappear safely.

Demon Cat

A demonic cat that appears at the end of hallways when viewing CCTV, causing instant sanity loss.

Counter

Keep CCTV viewing sessions short. If you see it, leave the camera immediately. The 20 sanity loss is unavoidable on sight.

Stalker (Disguised)

New behavior added in Harlow's Update: the Stalker can now appear at the check-in window disguised as a normal patient.

Counter

Photograph the suspicious patient. The Stalker will flee when photographed. Avoid eye contact to prevent sanity drain.

Slimebert (Hazmat Update)

Added in the Hazmat Team Arrives July 2026 update: a neon green oozing anomaly disguised in protective gear.

Counter

Use the Scanner tool or Hazmat Decontamination spray at the window. Do not open shutters until decontaminated.

Ceiling Eyes

Creepy glowing eyes that manifest on examination room ceilings, causing rapid Sanity drain if looked at directly.

Counter

Avoid direct eye contact. Look down at the floor and apply Eye Drops to restore lost Sanity immediately.

Lighting glitches are one of the game's environmental cues that something is wrong.

  • Flickering or dying lights usually mean an anomaly is active nearby or an emergency event is about to trigger. Treat it as a "pause and look" signal.
  • What to do: stop the admit flow, do a quick visual sweep of your surroundings, then re-check the CCTV feeds before you open the shutters.
  • Important: flickering lights do not tell you which anomaly is present — they only tell you one is near. You still need the full three-step check (visual → photo → CCTV) to confirm the patient at the window.
