Basement
Unlock the basement by completing the Doctor's Key quest line (Room 5 puzzle + Ratthew encounter). Exploring it earns the hidden 'Basement Explorer' achievement and connects to Barney's lore.
HR Archive
The staff-only HR Archive on the third floor (Admin area) requires a door code and hides lore documents about the hospital and its visitors.
Out-of-Map Area
Players have found an out-of-map area reachable through a glitch that hides a secret jumpscare button. Community videos document the exact route.
Secret Interactions
Liz NPC Photo Quest & Liz's House
Starting at Shift 7, Liz gives you 4 precise UV photo tasks. Warning: taking a photo of the wrong patient permanently fails the questline! Complete all 4 targets to trigger the Shift 20 invitation to Liz's House.
Dr. Harlow Secret Interaction
Give Dr. Harlow a Mint Tea ($100 from the Supplies Shop) to unlock his special lore dialogue about the hospital's visitors — a hidden achievement.
Ceiling Eyes Taming
You can tame the Ceiling Eyes ceiling-dweller by using Eye Drops on it, turning a threat into a tool.
No-Weapon & Trash Tricks
Kill anomalies without weapons using environment interactions, and throw fainted (unconscious) patients into the trash to avoid penalties.
CCTV Shooting
Security and Secret Agent players can shoot anomalies while watching through CCTV cameras — a powerful safe kill method.
