Emergency events trigger randomly during night shifts and will kill patients or drain your Sanity if ignored.

1. Death Ritual Event

Occurs in patient rooms randomly starting from Shift 2 onwards.

Hazard Description: A patient floats into mid-air above their hospital bed, surrounded by floating ritual candles. A dark chant audio plays and a red countdown timer begins. If the timer expires, the patient is sacrificed, causing a massive Sanity drop and automatic fail penalty.

How to Stop the Ritual

  • Method A (Standard): Sprint into the room and press E on every lit candle in the room. You must extinguish all candles within 30 seconds.
  • Method B (Fast / Pro Strategy): Equip the Taser and shoot the levitating patient once. The electrical shock breaks the spell instantly without needing to touch candles.

2. Bed Monster (Monster Under the Bed)

Hides under patient cots and emits a pulsing red zone.

Hazard Description: Shadows form under the bed frame and a dark red glowing circle spreads across the floor. Standing in the red zone rapidly drains your Sanity bar and risks pulling you under.

How to Counter & Banish

  • Maple Syrup Counter: Hold Maple Syrup (bought from Shopkeeper Nurse) and step into the red circle. The Bed Monster will consume the syrup and despawn immediately, giving you an achievement bonus!
  • Evasion: If you do not have syrup, jump over the bed frame to avoid touching the floor zone while checking patient vitals.

3. Room Fires & Spontaneous Combustion

Sparks ignite in patient rooms or directly on patient bedding.

Hazard Description: Alarm sirens sound and black smoke fills the hallway. Fire will spread across curtains and equipment, causing severe patient injury if not put out quickly.

Extinguisher SOP

  • Equip the Fire Extinguisher and aim directly at the base of the flames until the gauge drops to 0%.
  • If playing as the Firefighter Class, your passive fire suppression speed is doubled and extinguishers never overheat.
  • After putting out the fire, check burned patients and apply ointment to restore health.

4. Ambulance Rush Wave Event

A siren, a banner, and a sudden flood of patients from Shift 4 onwards.

Hazard Description: Starting from Shift 4, an ambulance can roll up to the hospital entrance with a rush wave of multiple patients at once. You will hear a siren, see a banner at the top of the screen, and the corridor suddenly fills with new beds. Multiple Critical Patients can arrive at the same time, and their 120-second timers can overlap if you hesitate.

Triage & Team Strategy

  • Slow is smooth, smooth is fast: don't panic-dash between beds. Read each patient's status bar, then act in one pass.
  • Prefer rejecting: if a patient has a low-value or repetitive condition and the queue is overflowing, rejecting it is better than letting a Critical Patient expire.
  • Assign roles in advance: one player on Shutter + Camera recon to spot anomalies, two fast runners handling the beds, and one combat float guarding against Stalker and Slime spawns mid-wave.
  • Pre-stock before the wave: Coffee (Sanity), Cola (stamina), and a Taser (anomaly insurance) in your quick-slots.

After the Wave

  • Drink coffee immediately to recover the Sanity burned during the rush.
  • Sweep the corridors for Slime and lingering anomaly shadows.
  • Run a quick DNA audit on every treated bed so no patient gets double-charged or forgotten.

5. Room 1 Gas Leak & Oxygen Crisis

New in August 2026: Toxic hazard, active oxygen pumping rhythm, and ghost prevention.

Hazard Description: Green toxic fumes burst from the ventilation shaft in Room 1, causing the patient's oxygen gauge to rapidly plummet toward zero. If oxygen reaches 0%, the patient suffocates and instantly dies, spawning a hostile Ghost in the wing and deducting team Sanity.

Pumping Minigame Mechanics & Rhythm

  • Controlled Pumping Cadence: Interact with the bedside respirator unit (E). Tap rhythmically (~2 taps per second) rather than spam-clicking. Over-pumping triggers an Overheat Jam, locking the pump for 3 critical seconds.
  • Green Safe Zone Maintenance: Keep the gauge above 80% until the automated exhaust ventilation timer expires (~35 seconds).
  • Dual-Hazard Arbitration: If a fire and gas leak trigger simultaneously, extinguish the flame first (fire deals instant burst damage), then tag-team the oxygen pump.
  • Emergency Wheelchair Evacuation: If gas density makes the room toxic to staff, place the patient on the lobby Wheelchair and wheel them to an empty bed in Room 3 or 4.
